Personal Data Collection Agreement
The National Synchrotron Radiation Research Center (NSRRC) collects, processes and uses the personal data provided by the undersigned for business needs, hereby informing the undersigned of the following matters in accordance with the provisions of the Personal Data Protection Act (the “PDP” Act). The undersigned is required to carefully read the following:
I. The purpose of personal data collection: For the needs of handling NSRRC's personnel management, information security management system, employee health checks, guest houses, and other related business, as well as for the needs of meeting the business requirements stipulated in the NSRRC's regulations.
II. The type of personal data collection: Personal identification data, such as name, national identification number, telephone number, mobile phone number, correspondence and household registration addresses, e-mail address, service unit, and job title.
III. The duration, territory, object and method for using the personal data:
(I) The duration: The duration for maintaining the personal data collected for the specific purpose conforms to the retention period in accordance with the provisions of the relevant laws and regulations, the contractual agreement, or the needs for performing NSRRC's business.
(II) The territory: The personal data is applicable in the Republic of China.
(III) The object: The personal data is applicable by related units, such as Administrative Division and Radiation & Operation Safety Division of NSRRC.
(IV) The method: The personal data is applicable with methods through automated or non-automated means.
IV. In accordance with the provisions of the Personal Data Protection Act, the undersigned may exercise the following rights regarding the personal data retained by NSRRC:
(I) To make an inquiry of and to review the personal data;
(II) To request copies of the personal data;
(III) To request for supplementation or correction of the personal data;
(IV) To request for cease of data collection, processing or use; and
(V) To request for data deletion.
However, the undersigned's requests may not be entertained in accordance with the provisions of the relevant laws and regulations, the contractual agreement, or the needs for performing NSRRC's business.
V. The impact on the rights and interests of the undersigned for not providing personal data:
The undersigned may opt to provide the relevant personal data or not. Where the undersigned refuses to provide the relevant personal data, NSRRC will not be able to carry out the necessary review and processing operations; therefore, may not be able to provide relevant services for the undersigned.
