NSRRC Preventative Measures for Novel Coronavirus (COVID-19) Pandemic
 (updated on 2021/05/14, effective from 2021/05/14)


* NSRRC will uphold any and all measures announced by Taiwan Centers for Disease Control (CDC) when tackling conditions not addressed in this document.

Entry & Exit Control
1. NSRRC staff:
While continuing to operate as usual, make sure to follow the attendance rules specified for the pandemic and the CDC's preventative measures for COVID-19.
2. Users, visitors, external technicians/constructors, and outsourced staff holding a temporary badge: 
- Enter and exit the NSRRC campus through the main gate ONLY. Before entering the campus, it is mandatory to have the temperature taken by security guards, and to fill out the "Health Declaration Form" (real-name system employed), and to obtain a visitor pass. Access to campus is not permitted if a fever or signs of a respiratory disease are detected. 
- The services of shuttle buses, taxies (including gypsy cabs), and food delivery can ONLY be done at the main gate if the drivers choose not to fill out the "Health Declaration Form" (real-name system employed).
3. Non-NSRRC staff who dine in NSRRC cafeteria:
Enter and exit the NSRRC campus through the main gate ONLY. Before entering the campus, it is mandatory to have the temperature taken by security guards, to fill out the "Health Declaration Form" (real-name system employed) and to obtain a visitor pass. Access to campus is not permitted if a fever or signs of a respiratory disease are detected.

Personnel Access Control: for users, sub/contractors, visitors etc.
1. Must provide proof of completing 14 days of quarantine and an additional 7 days of self-health management before entering NSRRC if entering Taiwan from foreign countries/regions. The 14 days are counted from the next day of arriving in Taiwan.
2. Must present the proof a self-pay COVID-19 RT-PCR test done after the quarantine and self-health management periods, with a negative result.
3. No guided tour to NSRRC will be scheduled before 2021/06/08. In response to the fast-changing situation that COVID-19 presents to us, NSRRC reserves the right to reschedule or cancel any guided tour that is approved or scheduled.


Note:
1. NSRRC reserves the right to deny the entry, or cancel the beamtime allocated to those who fail to comply with the preventative measures. NSRRC shall not be responsible or liable to anyone for any financial or other loss due to their failure to comply with applicable COVID-19 preventative measures.
2. Those who are required home quarantine can contact CDC for a collective quarantine facility at their own cost if they do not have a proper accommodation for quarantine. Those who are required self-health management should find their accommodation. NSRRC will not provide accommodation service to them.

Attachment: Information on Self-pay COVID-19 Test

Applicable to
1. NSRRC staff or new employees who require cross-border travel for work.
2. Venders/Users who enter Taiwan from foreign countries/regions.

Instruction
1. A member from the Group/Office/Endstation that requests the above person must be assigned as a contact person.
2. Once the 14-day quarantine and the 7-day self-health managment end, please go to Hsinchu Mackay Memorial Hospital or China Medical University Hsinchu Hospital for "Self-pay COVID-19 Testing" during the 7-day self-health management period.
(1) When filling out the attached application form, tick "Others" for the (application) Reason.
(2) Please bring your ID card/passport/resident certificate, and the completed application form when going  to ER for express COVID-19 testing. Non-citizens must be accompanied by the assigned contact person.

*If the test is not done before the above specified time, its report shall be available within 24 hours.

3. NSRRC will pay for the testing if staff or new employees require cross-border travel for work. Vendors and users have to do the testing at your own expenses.
4. Please bring your ID card/passport/resident certificate when going to the ER cashier to claim your test report.
5. Non-citizens should have the assigned contact person claim the test report for you. The contact person needs to bring the documents listed in 4, the authorization letter, and his/her ID card.
6. The contact person must inform his/her division head of the test result ASAP in order to pass the information on to the Epidemic Prevention Team. Please snapshot or scan the test report, and email it to safety@nsrrc.org.tw.
